BACKGROUND: Serotonin transporter promoter genotype appears to increase risk for depression in the context of stressful life events. However, the effects of this genotype on measures of stress sensitivity are poorly understood. Therefore, this study examined whether 5-HTTLPR genotype was associated with negative information processing biases in early childhood. METHOD: Thirty-nine unselected seven-year-old children completed a negative mood induction procedure and a Self-Referent Encoding Task designed to measure positive and negative schematic processing. Children were also genotyped for the 5-HTTLPR gene. (...) RESULTS: Children who were homozygous for the short allele of the 5-HTTLPR gene showed greater negative schematic processing following a negative mood prime than those with other genotypes. 5-HTTLPR genotype was not significantly associated with positive schematic processing. LIMITATIONS: The sample size for this study was small. We did not analyze more recently reported variants of the 5-HTTLPR long alleles. CONCLUSIONS: 5-HTTLPR genotype is associated with negative information processing styles following a negative mood prime in a non-clinical sample of young children. Such cognitive styles are thought to be activated in response to stressful life events, leading to depressive symptoms; thus, cognitive styles may index the "stress-sensitivity" conferred by this genotype. OBJECTIVES: Early-emerging, temperamental differences in fear-related traits may be a heritable vulnerability factor for anxiety disorders. Previous research indicates that the serotonin transporter promoter region polymorphism is a candidate gene for such traits. METHODS: Associations between 5-HTTLPR genotype and indices of fearful child temperament, derived from maternal report and standardized laboratory observations, were examined in a community sample of 95 preschool-aged children. RESULTS: Children with one or more long alleles of the 5-HTTLPR gene were rated as significantly more nervous during (...) standardized laboratory tasks than children who were homozygous for the short alleles. Children homozygous for the short alleles were also rated as significantly shyer, by maternal report, than those with at least one copy of the long allele of the 5-HTTLPR gene. CONCLUSIONS: This study extends the literature linking the short alleles of the serotonin transporter promoter region polymorphism to fear and anxiety-related traits in early childhood and adulthood, and is one of very few studies to examine the molecular genetics of preschoolers' temperament using multiple measures of traits in a normative sample. DOUGHERTY The Catholic University ofAmerica Washington, D.C. I T HE MEDIEVAL problem of individuation is not the contemporary problem of "individuals" or "particulars" discussed by P. F. Strawson, J. W. Meiland, and others.1 In a certain sense the problem of individuation originates with Parmenides, but it is Plato's philosophy of science that bequeaths the problem (...) to Aristotle and to his medieval commentators. Its solution in Aquinas is not that of Aristotle, nor is it that of Scotus or Suarez. Aquinas will distinguish between the problem of individuation and what we may call the problem of "individuality" or the problem of "subsistence." The solution to both will draw upon many Aristotelian distinctions but will incorporate key elements of St. Thomas's own metaphysics, including the real distinction between essence and existence and his doctrine of participation. It is Maritain's appropriation of St. Thomas's metaphysics that enables him to produce a realistic philosophy of science, one that he offers as compatible with contemporary scientific enquiry. It also enables him to develop a theory of person and personality. But the story begins with Plato. Although Plato's theory of knowledge may appear fanciful to the modern reader, his analysis of scientific knowledge contains a basic set of observations whose truth remains uncontested even 1 Cf. Things may be particular, but when we consider them as objects of enquiry, the intellect focuses upon the form taken as an exemplar. In Plato's explanation things belong to their various kinds by participating in incorporeal, eternal, and unchangeable archetypes. From a realist's vantage point the problem may be stated simply: Since things are singular, how is it that we intellectually apprehend them as universal? Aristotle's solution is well known and it is one adopted and amplified by St. Thomas. Universals are abstracted from singular things. No one would present Maritain as a medievalist, but, as an interpreter of Aquinas, he has wielded considerable influence in the United States and in Latin America. Many have come to St. Thomas under his tutelage. His knowledge of Aquinas is extensive and is drawn upon throughout his lifelong work, but perhaps nowhere more than in his philosophy of science and in his discussions of the person. The primary text for Thomas's doctrine of individuation is his commentary on Boethius's De Trinitate, where he discusses the division and methods of the sciences. Maritain's philosophy is indebted mainly to his reading of Thomistic texts, but he draws heavily, as well, on the works of his contemporaries, Reginald Garrigou-Lagrange and Louis Geiger, and on those of the classic commentators on Thomas, Cajetan, Sylvester of Ferrara, and John of St. Thomas. Though employing St. Thomas, Maritain is always a man of the twentieth century. The standard approach to protecting privacy in healthcare aims to control access to personal information. We cannot regain control of information after it has been shared, so we must restrict access from the start. This ‘control’ conception of privacy conflicts with data-intensive initiatives like precision medicine and learning health systems, as they require patients to give up significant control of their information. Without adequate alternatives to the control-based approach, such data-intensive programmes appear to require a loss of privacy. This paper (...) argues that the control view of privacy is shortsighted and overlooks important ways to protect health information even when widely shared. To prepare for a world where we no longer control our data, we must pursue three alternative strategies: obfuscate health data, penalise the misuse of health data and improve transparency around who shares our data and for what purposes. Prioritising these strategies is necessary when health data are widely shared both within and outside of the health system. (shrink)

Some philosophers have thought that life could only be meaningful if there is no God. For Sartre and Nagel, for example, a God of the traditional classical theistic sort would constrain our powers of self-creative autonomy in ways that would severely detract from the meaning of our lives, possibly even evacuate our lives of all meaning. Some philosophers, by contrast, have thought that life could only be meaningful if there is a God. God and the Meanings of Life is interested (...) in exploring the truth in both these schools of thought, seeking to discover what God could and couldn't do to make life meaningful (as well as what he would and wouldn't do). Mawson espouses a version of the 'amalgam' or 'pluralism' thesis about the issue of life's meaning – in essence, that there are a number of different legitimate meanings of 'meaning' (and indeed 'life') in the question of life's meaning. According to Mawson, God, were he to exist, would help make life meaningful in some of these senses and hinder in some others. He argues that whilst there could be meaning in a Godless universe, there could be other sorts of meaning in a Godly one and that these would be deeper. (shrink)

Recent highly publicized privacy breaches in health care and genomics research have led many to question whether current standards of data protection are adequate. Improvements in de-identification techniques, combined with pervasive data sharing, have increased the likelihood that external parties can track individuals across multiple databases. This paper focuses on the communication of identifiability risks in the process of obtaining consent for donation and research. Most ethical discussions of identifiability risks have focused on the severity of the risk and how (...) it might be mitigated, and what precisely is at stake in pervasive data sharing. However, there has been little discussion of whether and how to communicate the risk to potential donors. We review the ethical arguments behind favoring different types of risk communication in the consent process, and outline how identifiability concerns can be incorporated into either a detailed or simplified method of communicating risks during the consent process. Amongst other countries, the Netherlands currently allows euthanasia, provided the physician performing the procedure adheres to a strict set of requirements. In 2016, Second Chamber member Pia Dijkstra submitted a law proposal which would also allow euthanasia without the reason necessarily having any medical foundation; euthanasia on the basis of a completed life. The debate on this topic has been ongoing for over two decades, but this law proposal has made the discussion much more immediate and concrete. This paper considers (...) the moral permissibility of Pia Dijkstra’s law proposal, focusing on the ethics of the implementation Dijkstra describes in her proposal. I argue that, at present, Dijkstra’s law proposal is unsuitable for implementation, due to a number of as of yet unaddressed problems, including the possible development of an ageist stigma and undue pressure on the profession of end-of-life coordinator. Perhaps adequate responses can be conceived to address these issues. However, the existence of a radically different, yet currently equally unacceptable position regarding the implementation of euthanasia for a completed life as proposed by fellow party member Paul Schnabel suggests it may be difficult to formulate an ethically acceptable implementation for this, in principle, ethically acceptable concept. This paper argues that the Food and Drug Administration’s (FDA) policy for health and wellness apps is ethically problematic. Currently, the FDA does not regulate health and wellness apps that are not intended for medical use. As a result of this hands‐off policy, preventing harm to consumers is left primarily to developers and app marketplaces. We argue that the FDA’s duties to prevent harm and maintain accountability to the American public require that they play a much stronger role. This chapter introduces the descriptive analogue of self-location. It argues that if a language contains predicates that apply to the properties it exemplifies, and it contains reflexive expressions that identify those properties, we have the makings of self-describing sentences that do for its descriptive vocabulary what self-locating acts do for spatial vocabulary.

This chapter argues that the formal requirements on self-describing media shed light on two elusive questions in the philosophy of mind. The first is a question that Dretske raised in Naturalizing the Mind: why do we have conscious access to the intrinsic properties of experience? In his terms, the question is: what is experience for? The second is a question that has hounded philosophy of mind since Brentano: in what sense, if any, is thought intrinsically intentional? What is the property (...) that some states have of pointing, or purporting to point, beyond themselves? Dual antiviral therapy with pegylated interferon-2a and ribavirin only reaches sustained virological response rates of ~30% after liver transplantation. With the approval of viral NS3/4A (...) protease inhibitors telaprevir, boceprevir, and simeprevir and the NS5B polymerase inhibitor sofosbuvir, combination therapy offers new therapeutic options for HCV-infected patients, resulting in considerably higher sustained virological response rates in the nontransplant setting. Case presentation: We report three cases of TVR-based triple antiviral therapy in HCV genotype 1 reinfected patients after liver transplantation, of whom a 57-year-old Caucasian female and a 43-year-old Caucasian male were therapy naïve, and a 49-year-old Caucasian male patient was pretreated ineffectively. After 4 weeks of therapy, viral load decreased one to three log10 and became negative in weeks 6 to 8 in the therapy naïve patients. The pretreated patient showed a negative viral load in week 4. TVR was administered over 12 weeks, 750 mg thrice daily. Doses of immunosuppression with cyclosporine were reduced four to six fold. Initial peg-IFN and RBV doses ranged from 135–180 µg/week and 800–1,200 mg/day, according to the patient's body weight. Doses of peg-IFN and RBV were adapted to 90–135 µg/week and 400–800 mg/day after 2 to 12 weeks of protease inhibitor therapy. Dual therapy was continued for 36 weeks with total treatment duration of 48 weeks in the therapy naïve patients leading to a sustained virological response 12 weeks after the end of therapy. In the pretreated patient a breakthrough was detected in week 24 and therapy was discontinued. Overall, antiviral therapy was well tolerated. Side effects included dysgeusia and anemia leading to erythropoietin application and blood transfusions. Conclusion: This case series emphasizes that triple therapy with TVR is an efficient treatment for therapy naïve HCV genotype 1 reinfected patients after liver transplantation. While the humanist project helped itself through its conquest of the human sphere, it has served to upset the ecological balance and jeopardize sustainability. The theme of this volume is natural beauty, landscape and the arts. The first question for a philosopher to ask is what does philosophy have to say now particularly about natural beauty. I emphasize now, because, as is well known, historically philosophers, for example, Plato and the eighteenth-century British, and especially Scottish, philosophers, were interested in the topic of beauty. At the present day there has also been some revival of interest in this subject, but when it comes to what (...) philosophers past and present have had to say about beauty, I am doubtful how much relevance it has to the question of how we should think about natural beauty. We must be careful then in an inquiry into natural beauty not to begin by taking up some question within the traditional philosophy of beauty – such as for instance whether beauty is a real quality, or whether judgments of beauty are subjective or objective – for it is difficult to see what such metaphysical lucubrations could do for the question in hand, other than offering, what is not to be underestimated, an innocent form of academic pleasure.In any case, the modern world takes a short cut with the question of beauty: everyone, unless having a distinct reason for believing the contrary, such as a specific religious faith, unhesitatingly takes it for granted that an extreme subjectivism and relativism must hold sway. Wuilleumier identifies the Galaesus with the Citrezze or Giadrezze, a small stream running into the north side of the Mare Piccolo, about two miles from the channel on the west side of the citadel of Tarentum which connects the Mare Piccolo with the sea. This identification, which has been often repeated since Lenormant's time and spread beyond the narrow bounds of pure scholarship by the writings of George Gissing , Norman Douglas , and (...) David Randall-Madver , is likely to hold the field by virtue, of Wuilleumier's support. But it is irreconcilable with the only ancient evidence on the position of this river, given in the account of Hannibal's movements in 212 B.C. (shrink)
